A DMCA counter-notification must include the following:

1. Your physical or electronic signature.
2. Identification of the removed or disabled material and the location where it appeared before it was removed or access was disabled.
3. A statement under penalty of perjury that you have a good faith belief the material was removed or disabled due to a mistake or misidentification.
4. Your name, address, phone number, and a statement consenting to the jurisdiction in which you are located.
5. A statement agreeing to accept service of process from the person who filed the original infringement notice or their agent.
